Founded in 1999 by Pernille Ostberg and Janice Williams, Matrix Home Care is dedicated to providing holistic home care that transcends medical needs. Our mission is to enhance clients’ quality of life by addressing clinical, social, and emotional health through our unique Champion Program. We assess functional abilities and evaluate Activities of Daily Living (ADLs). Our team then provides coordination of care and professional caregivers.

We use a tiered, team approach led by a Champion, someone who, on a management level, oversees all the non-clinical needs of the client and the family.
Develop a Customized Care Plan
Comprehensive evaluation of an individual’s physical, emotional, and social needs, conducted to create a customized care plan for the well-being of the client.
Assessment of the Home
Comprehensive evaluation of an individual’s home environment to assess challenges, safety and make recommendations as part of a care plan.
Start Services
A caregiver is selected for the client based on their needs and wants and the care plan is thoughtfully implemented. Services are provided according to your schedule.
